Explore the proposed modelALOC Safe Discharge Health aims to support safer, more coordinated transitions from hospital to home and community-based care.
We recognise the challenges faced by hospitals when patients are medically ready to leave hospital but require additional care, practical support or coordination before returning home safely.
Our proposed service model brings together healthcare organisations, care workers, community services, patients and families through a coordinated approach to discharge support.
Seven proposed service areas designed to help participating organisations address barriers, coordinate support and improve the transition home.
Support communication and coordination between hospitals, community services and care providers to help address barriers to discharge.
Develop a digital platform through which participating hospitals can view available, appropriately vetted care workers and request support according to a patient’s assessed needs.
Enable authorised hospital staff to submit requests detailing the support required, anticipated discharge date, relevant care requirements and other essential arrangements.
Facilitate access to suitable home-based support, subject to individual assessment, workforce availability, service scope and agreed arrangements.
Support appropriate coordination for patients whose discharge requires input from multiple professionals or services, including older people and those living with frailty.
Explore appropriate coordination, administrative and consultancy services relating to NHS Continuing Healthcare, All-Age Continuing Care and associated pathways, within agreed professional responsibilities.
Develop training and consultancy opportunities in discharge planning, care coordination, patient flow and service improvement.
We aim to develop a system that helps authorised hospital teams coordinate discharge support and helps participating care workers manage appropriate assignments.
